RE: Mandatory Preregistration Requirements

Welcome to UTMB Student Health and Student Counseling. We look forward to working with you to achieve optimal health. In order to accomplish this goal, we need your assistance in completing your preregistration forms.

The orientation presentation below will provide information about our services. You will also find Immunization Requirements for enrollment as a student at Galveston College and an Immunization Record form. This form must be completed and signed by your primary care provider or a nurse at the clinic where you receive your immunizations or attach copies of your immunizations to the completed form. The immunization requirements comply with the State of Texas, the Texas Department of Health, and the CDC. You must meet ALL identified requirements. Students must complete all immunizations prior to enrollment.

Immunizations are available at Student Health after acceptance. You may call Student Health for an appointment. Please remember that you must have received an acceptance letter to be eligible to come to Student Health for your immunizations.

Please mail or e-mail your completed and signed Immunization Record form. Our email and physical address is located at the bottom of the immunization record form. Please return the form by December 1 if enrolling for the spring semester, April 1 if enrolling for the summer semester and August 1 if enrolling for the fall semester. Please remember that immunizations are mandatory for your enrollment at Galveston College. If you have any questions, please feel free to contact Student Health and Counseling at (409) 747-9508.
