Student Organization Annual Registration Packet - Form used by current student organizations needing to re-register.

New Student Organization Registration Packet - Form to be used by newly formed student organizations that have not been registered before.

SGA Request for Funding Form - Form used to request activity funds from the Student Government Association Budget Committee. Budget Committee meets each Monday and form must be turned into the Office of Student Life by 1:00 pm the of the previous Friday in order to be on the Agenda. Also, all requests must be made to the Budget Committee at least 3 weeks prior to the date of the event.

SGA Request for Travel Funding Form - Form used to request travel funds from the Student Government Association Budget Committee. Also, all requests must be made to the Budget Committee at least 3 weeks prior to the date of the event.

Student Organization Request for Reimbursement Form - Form used to request reimbursements for Budget Committee authorized expenses. This form and all original receipts, airline boarding passes and any other invoices that are applicable must be turned in together no later than 2 weeks after the event takes place.

Activity Authorization Form - Form used by student organizations to record a meeting that will have a speaker from off campus or to request space for a special activity.

SGA Special Resource Funding (SRF) Request Form - From used by class officers to request funds from the budget committee out of their special resource funding.
