Dear Student:

Welcome to UTMB Student Wellness.  We look forward to working with you to achieve optimal health during your tenure as a student.  Working toward that goal we need your assistance in completing your preregistration health screening.

Enclosed you will find Immunization Requirements for enrollment as a student at UTMB and an Immunization Record form which must be completed and signed by your primary care provider or a nurse at the clinic where you receive your immunizations.  The immunization requirements comply with the State of Texas Higher Education Mandatory Immunization Requirements and the recommendations of the Texas Department of State Health and the CDC.  You must meet ALL identified requirements.  Students may not participate in direct patient care until all immunization requirements are complete.  Please note your TB skin test (PPD) must be within 6 months of enrollment.

Immunizations are available at Student Wellness after acceptance prior to your first semester of enrollment for the rates enrolled students are charged plus an administrative fee of $10 per immunization. You may call Student Wellness for an appointment at 409-747-9508.  Please remember that you must have received an acceptance letter to be eligible to come to Student Wellness for your immunizations.

Please return your completed and signed Immunization History form by December 1 if enrolling for the spring semester, April 1 if enrolling for the summer semester and July 1 if enrolling for the fall semester.  Please remember that it is your responsibility to ensure that you complete the immunization requirements.Please feel free to contact Lorita Sowell at the number listed below if you have any questions.  The Student Wellness staff looks forward to meeting you during orientation.
