As of June 30, 2014 the Department of Internal Medicine embarked on a redesign of our inpatient services and outpatient general medicine services. Our Internal Medicine Redesign is centered upon 6 guiding principles.

  • The model promotes patient and family centered care
  • The model supports effective communication with patients and families
  • The model supports efficiency in the delivery of service to patients
  • The model creates a structure for monitoring and improving patient outcomes
  • The model supports resident education in communication and interdisciplinary team based care
  • The model improves consistency and continuity of care across the continuum

Service Changes - Inpatient

  • Inpatient practice from each team will be geolocalized
  • Providers will be on unit 24/7 to admit patients and provide cross coverage.
  • Transfer of care becomes decentralized and involves the patient (at the bedside)
  • Unit staff will be able to contact patient care teams easily.
  • Structured transfer of care at the bedside with faculty present to ensure continuity of the plan of care.
  • Faculty on service for 2 week blocks and protected from outpatient responsibilities
  • 2 Noctunalists will be hired to increase faculty supervision at night, shift starting at 5pm.

Service Changes - Outpatient

  • Resident and faculty run clinic together, utilizing same space and staff.
  • Acute care and same day care is built into templates to ensure access to care.
  • Patients belong to a PCP and the team, allowing for continuity of care at any clinic visit.”

Our hospitalist program consists of Leah Low, MD and Ahmed Morsy, MD. In addition to teaching on the inpatient ward services, they also supervise the medicine consult service.
