Medicare has extended the Telehealth Flexibilities until Sept. 30, 2025. Other payers are starting to make decisions as to how they would like to handle telehealth. It is very important that when completing a telehealth visit, the TELEHEALTH NOTE is used. Using the telehealth note will bring up the proper documentation elements and add the appropriate telehealth modifier and place of service.
The following documentation elements are required for telehealth.
- Patient Consent to a Telehealth Visit
- The location of both the provider and the patient
- The communication method (i.e. audio/video or audio only)
- If audio only was used, document if patient is not capable of or does not consent to video
Behavioral Health
The requirement for an in-person visit for behavioral health is delayed until Oct. 1, 2025. Also, as of Jan. 1, 2025, the documentation requirements for Telehealth services were updated to support mental/behavioral telehealth services, which means the questionnaire below needs to be completed in EPIC to distinguish them from other Telehealth services.
