This innovative project addressed a critical gap in disaster preparedness for older adults, a population often at heightened risk during hurricanes and other natural disasters. Through the development and implementation of a community-based quality improvement model, the team strengthened local resilience and enhanced safety for older residents across Galveston County.
The study’s success highlights the pivotal role of Advanced Practice Providers in translating evidence-based strategies into real-world community impact, improving outcomes, fostering collaboration, and advancing age-friendly care beyond the clinic walls.
