Testosterone Replacement Therapy: What Men Need to Know

In observance of Men's Health Month, we are highlighting an issue that continues to attract interest and questions among male patients: testosterone replacement therapy (TRT).

Samuel Kahiu, MSN, APRN, FNP-C, Nurse Practitioner at UTMB Health South Shore Pediatric and Adult Primary Care, shares his expertise and clinical insight on this topic.

Testosterone is an essential hormone that contributes to men's overall health, impacting aspects such as energy levels, mood, muscle mass, bone density, libido, and sexual function. Although testosterone levels tend to decrease naturally with age, some men experience symptoms that significantly affect their quality of lif

He emphasizes that experiencing fatigue or decreased energy levels does not necessarily indicate low testosterone. While symptoms such as fatigue, diminished libido, erectile dysfunction, muscle loss, difficulty concentrating, irritability, and decreased motivation can be linked to testosterone deficiency, they may also be associated with other medical conditions or lifestyle factors.

Diagnosing low testosterone involves more than assessing symptoms alone, Mr. Kahiu explains. Laboratory testing is essential, usually starting with a blood sample collected in the morning when testosterone levels are at their peak. If the initial results indicate low levels, additional testing is often conducted to confirm the diagnosis prior to discussing treatment options.

For men exhibiting both symptoms and persistently low testosterone levels, testosterone replacement therapy may be a suitable treatment option. The therapy can be delivered through various methods, including topical gels, injections, patches, and implanted pellets. The most appropriate choice typically depends on patient preferences, lifestyle factors, cost considerations, and clinical assessment.

When discussing the potential benefits of TRT, Mr. Kahiu notes that many patients report improvements in energy, mood, libido, and overall well-being. Some men also experience increases in muscle mass and positive changes in body composition. However, he emphasized that testosterone therapy should not be regarded as a universal solution or a shortcut to improved health. Maintaining a balanced diet, engaging in regular physical activity, obtaining sufficient sleep, and managing chronic health conditions are essential elements of overall wellness.

He also emphasizes the importance of understanding the potential risks associated with therapy. Testosterone replacement therapy necessitates continuous monitoring, including regular laboratory tests and follow-up appointments. Possible side effects may include increased red blood cell counts, acne, fluid retention, and the potential worsening of certain underlying conditions.

Additionally, testosterone therapy may substantially decrease sperm production and affect fertility, which should be considered by men considering future family planning.

The primary insight Mr. Kahiu shares is the importance of open communication between patients and their healthcare providers. It is common for men to assume that symptoms such as fatigue, decreased energy, or diminished sexual function are just inevitable aspects of aging. While some age-related changes are normal, others may indicate an underlying medical condition that warrants further assessment.

As Men’s Health Month highlights, prioritizing preventive care and addressing health concerns proactively can lead to positive outcomes. Testosterone replacement therapy may be advantageous for certain individuals; however, accurate diagnosis, personalized treatment plans, and regular monitoring are critical to ensuring safety and effectiveness.

A sincere thank you to Sam Kahiu, MSN, APRN, FNP-C, for sharing his expertise and clinical insight on testosterone replacement therapy. By dedicating time to educate both patients and colleagues, he exemplifies the crucial role APPs play in promoting preventive care, enhancing health outcomes, and advancing men's health within our community.
