VANCOMYCIN, serum (087-0452)

Specimen Requirements:
Collection:

Routine venipuncture. Draw trough levels immediately before dose; peak levels 0.5 to 1.0 hour after the end of a 30- to 60-minute infusion or one hour after IM dose.  collect 7mL of blood in Red-topped tube. (Min.: 3mL of blood).

DO NOT draw blood in SST/gel separator vacutainer tube.

Storage/Transport:

Delivered to Sample Management within 2 hours of collection.   

Specimen Preparation:

Allow specimen to clot completely at room temperature. Centrifuge to separate serum from cells within 2 hours of collection. Transfer serum from red-topped tube to labeled plastic transport tube. 

Stability:

Room Temperature for ≤2 hours, Refrigerated   for ≤3 days, Frozen for ≤14 days. (Avoid repeated freeze/thaw cycles).

Causes for Rejection:

Blood drawn in SST/gel separator vacutainer tube, incomplete and/or incorrect sample identification, improper storage/transport, gross hemolysis, icterus or lipemia.

Note:

If sample is obtained through the infusion set, flush the line thoroughly with saline before taking the blood sample. Request form must contain date and time of collection and time of last dose.
