Breast Biopsy (98305)

Specimen Requirements:

Per ASCO/CAP guidelines breast tissue must be fixed in formalin for a minimum of 6 hours to a maximum of 48 hours before undergoing HER2 testing. 

The time must be documented on the specimen container and in the clinical history in Epic Beaker.  

Container:

Pre-filled containers with 10% neutral buffered formalin.

Storage/Transport:

Submit biopsy in 10% neutral buffered formalin in a container labeled with the patient's first and last names, unit history number and specimen identification.

Causes for Rejection:

Electronic order must be submitted in Beaker and must contain the specimen source, time of the biopsy and the time submitted in formalin, clinical history and diagnosis this can be cause for rejection or delay in processing of the specimen.  Along with contaminated containers.
