Brian is a 6-month-old infant who has been a patient in the Children's Center for Restorative Care for 3 months while his severe bronchopulmonary dysplasia is stabilized. He spent the first 3 months of life in the neonatal intensive care unit. He has been critically ill on numerous occasions, requiring re-intubation, and finally has stabilized with a trache collar and humidified oxygen at 40% during the day. He also has a feeding gastrostomy through which he receives drip feeding of an enriched elemental formula. He is on four inhaled medications and two enteral medications. His developmental level is 4-6 months. His family lives 150 miles from the medical center.

You have just started your 1-month rotation on the Children's Restorative Care Team, the hospital team which cares for special needs children.  Brain has been assigned to you as your patient for the next month. or until discharge. The Team meets to discuss discharge planning.

List six major issues which must be addressed before Brian can be discharged.
