Heart Failure
Readmissions

UTMB defines hospital readmission as patient admission to a hospital within 30 days after being discharged from an earlier hospital stay.

UTMB’s 30-day all-cause readmission rate for patients with heart failure has shown steady improvement, and we experienced a slight decrease in 2015. However, our average remains above our Academic Medical Center Peers' average, meaning our heart failure patients are readmitted more often. In 2015, over 22% of HF patients came back to UTMB within 30 days of being discharged, for an illness that may or may not have been related to their heart failure.
