The following questionnaire is part of our efforts to improve our services to UTMB students. Your responses are confidential and your participation is entirely voluntary.
UTMB Health Services does not have access to the raw response data. The Office of Institutional Effectiveness provides tabular summaries of the responses to UTMB Student Health on a periodic basis.
No personal identifiers are collected in the body of the survey and all data will be used in a form that will make it impossible to determine the identity of the individual responses.
That is, the survey responses will not be integrated, analyzed, or reported in any way in which the confidentiality of the survey responses is not absolutely guaranteed.
