Inquiries From Active TDCJ Inmates
Each inmate within the Texas Department of Criminal Justice system has a grievance process available for their use to process their healthcare inquiries. The goal is to resolve issues/ concerns involving healthcare at the lowest level and provide the fastest method of resolution. The following is a brief overview of the process:
- Inmate submits a Sick Call Request (SCR) to the facility medical department.
- If the response to the SCR does not resolve the inquiry, then the inmate may submit an I-60 form to the facility practice/operations manager or business manager.
- If the response to the I-60 is not satisfactory and/or does not resolve the inquiry, then a Step 1 Grievance form can be completed.
The I-60 and Step 1 Grievance are both processes that occur at the facility level
If the response to the Step 1 Grievance does not resolve the inquiry, then a Step 2 Grievance form can be completed. The Step 2 grievances are processed through the Central Grievance Office in Huntsville; thus, allowing a review by non-UTMB personnel. Inquiry From Unknown Sources
We redirect these inquiries to the inmate for his or her use of the unit-based informal resolution or grievance process. The option is available to email your inquiry to the Texas Department of Criminal Justice Office of the Independent Ombudsman.
Inquiry From a Relatives or a Family Members of the Inmate
UTMB-CMC Department of Quality Services, take our commitment to patient confidentiality seriously. As a result, we have established the following process to answer health-related inquiries from family members or patient advocates:
- Obtaining a release of information from the inmate allowing
the release of health-related information to the relative.
Contact the patient’s unit of assignment and request to speak to the medical department. Request that a release of information (ROI) form be completed by giving your name, address, phone number, and relationship to the patient. This form will be presented to the patient for signature and completed if the patient agrees to allow information sharing. The ROI is effective for 2 years but can be revoked at any time by the patient. - You may also request an ROI by contacting the UTMB-CMC Quality Services department below:
