Stepping into Leadership: A New Chapter for an Advanced Practice Provider

In today’s evolving healthcare landscape, the role of Advanced Practice Providers (APPs) continues to expand beyond direct patient care. Reflecting this progression, Allison Mitchell, DNP, APRN, AGNP-BC, a highly respected APP within the UTMB Division of Nephrology, has recently been appointed as the new Nephrology APP Lead.

This role not only acknowledges Dr. Mitchell’s clinical expertise and dedication but also positions them to influence strategic direction, support team development, and foster a culture of excellence within the division.

To gain insight into her vision, leadership philosophy, and objectives in this role, Dr. Mitchell graciously participated in a Q&A session.

Can you describe your journey to becoming the Lead APP in Nephrology, and what motivated you to take on this leadership role?

My journey to becoming the Lead Advanced Practice Provider (APP) in Nephrology at UTMB has been both fulfilling and driven by a deep-rooted passion for the field.  I have been with UTMB for seven years, and my interest in nephrology was sparked early on, as I was raised by a nephrologist and grew up immersed in the specialty.  

Throughout my APP training, I tried to move to another specialty, but with every project I completed, I kept finding myself in the nephrology text. This background instilled in me a profound understanding and appreciation for chronic disease management. 

I hold a Doctorate in Nursing Practice (DNP), which has equipped me with advanced skills to enhance patient care and optimize clinic processes. My previous experience in the business world, particularly in retail, has given me a unique perspective on process improvement and efficiency. This combination of clinical expertise and business acumen has fueled my obsession with improving clinic operations.

The rapidly growing population of patients with chronic kidney disease underscores the necessity for a collaborative approach to care. Managing this patient population cannot be the sole responsibility of physicians. Advanced Practice Providers (APPs) play a crucial role in delivering comprehensive care. My motivation to take on this leadership role stems from a desire to bridge the gap between APPs and MDs, fostering better communication and collaboration to ultimately improve patient outcomes.

As Lead APP, how do you ensure effective collaboration and communication among nephrology APPs, attending physicians, and other healthcare providers?

Effective collaboration and communication are cornerstones of high-quality patient care. As the Lead APP, I prioritize creating an environment where open dialogue and teamwork are encouraged. Here are some strategies I employ:

Regular Interdisciplinary Meetings: I attend regular meetings that include APPs and attending MD’s.  These meetings provide a platform for discussing patient cases, sharing insights, and addressing any concerns.  We are working to improve the frequency and quality of these meetings with a particular focus on value-based care.   

Clear Communication, Training, and Organization: Establishing clear and efficient communication channels is essential.  We are utilizing technology, protocols, training programs, and guidelines for seamless information exchange and learning.

Collaborative Quality Improvement Projects:  I am currently spearheading quality improvement (QI) projects that involve input and participation from the entire nephrology team. These projects not only enhance clinic processes but also strengthen team cohesion.

Mentorship and Support: I provide mentorship and support to APPs, fostering their professional growth and ensuring they feel valued and heard within the team.

What innovations or improvements have you introduced within your Nephrology team that have enhanced patient care or team efficiency?

Innovation and continuous improvement are integral to advancing patient care and team efficiency. Some of the key initiatives I have introduced include:

Quality Improvement Projects: I have led QI projects aimed at optimizing clinic processes. These projects have resulted in streamlined workflows with the goal of reducing wait times, improving patient satisfaction, and enhancing clinic outcomes.

Advanced Clinical Protocols: Implementing evidence-based clinical protocols has standardized care delivery, ensuring that all patients receive the highest quality of care.

Technology Integration: Leveraging technology, such as electronic health records (EHR) and AI (i.e. DAX), has enhanced our ability to monitor and manage patients with chronic kidney disease more effectively.

Interdisciplinary Training Programs: I have developed a nephrology APP training program that promotes interdisciplinary learning and collaboration, ensuring that all team members are well-equipped to provide comprehensive care.

Looking ahead, what are your key goals for the nephrology APP team, and how do you envision the role of APPs evolving within this specialty? 

Looking ahead, my key goals for the nephrology APP team include:

Expanding APP Roles: I aim to further expand the roles and responsibilities of APPs within the nephrology team, allowing them to take on more complex patient care tasks and leadership positions.

Enhancing Education and Training: Continuous education and training are vital. I plan to implement advanced training programs that keep our APPs at the forefront of nephrology care.

Strengthening Collaboration: Building even stronger collaborative relationships between APPs, physicians, and other healthcare providers will remain a priority. This includes fostering a culture of mutual respect and shared decision-making.

Advocating for APPs: I will continue to advocate for the recognition and utilization of APPs in nephrology, highlighting their essential role in managing the growing chronic kidney disease population.

The role of APPs in nephrology is evolving rapidly. As the demand for nephrology services increases, APPs will become even more integral to providing high-quality, patient-centered care. By embracing innovation, continuous improvement, and collaboration, we can ensure that our nephrology team remains at the cutting edge of patient care.
