J-1 Sponsor Letter - For all J-1 applicants, including extensions and transfers.
J-1 English Proficiency Attestation Form - Required for all J-1 applicants.
J-1 English Interview Form - Only required if relying on interview for evaluation of English proficiency.
J-1 Waiver Attestation Form - Required for all J-1 applicants.
J-1 Transfer Form - For J-1 visa holder coming to UTMB from another U.S. institution.
J-1 Short Term/Research Scholar Form - Comparison of the Short Term Scholar and Research Scholar categories.
Additional forms for J-1 applicants with medical degrees, medical licenses, or experience working as clinical physicians (whether or not they will be working as physicians at UTMB)
J-1 Incidental Patient Contact Letter - Required for applicants who will have incidental patient contact.
J-1 No Patient Care Letter - Required for applicants who will engage in no patient care at all.
