While hospitalized Brian has repeatedly developed problems with skin breakdown in the area of this gastrostomy.  This has been successfully treated with a compounded cream (specifically prepared by a pharmacist) consisting of 4 gm cholestyramine in 30 g Aquaphor, 30 gm Zinc oxide cream and 30 gm Burrow's solution.  This compounded preparation is prepared at the university pharmacy.  You would like to allow Brian's mother to have access to this cream after she goes home.

How would you best go about obtaining this medication from him?

  1. Write a prescription for Brian's mother to take to the out patient pharmacy in her home town.

  2. Write a prescription and fax it to the Case Manager who will locate a convenient pharmacy for the family to get prescriptions.

  3. Write a prescription for Brian's mother to take to the university outpatient pharmacy to be filled.
