NIH issues Guidance on FORMS-E Expiration Dates

Continue to Use Current Forms (FORMS-E) through May 24, 2020

NIH stated in November through NOT-OD-20-026 changes (FORMS-F) were coming to the application forms.  The following forms would include substantive changes (i.e., new/deleted/modified fields):

  • PHS 398 Career Development Award Supplemental Form
  • PHS 398 Cover Page Supplement
  • PHS Assignment Request form
  • PHS Fellowship Supplemental Form
  • PHS Human Subjects and Clinical Trials Information
  • SBIR/STTR Information

Many other forms would include only a new OMB expiration date. These new forms will be effective for all applications submitted for due dates on or after May 25, 2020.

In the meantime, per NOT-OD-20-047, researchers are told to continue to use FORMS-E if your due date is on or before May 24, 2020.  Specifically, if your application form has an expiration date of 12/31/2019:

  • SF424 R&R
  • SF424 R&R Multi-project
  • Research & Related Other Project Information
  • Research & Related Senior/Key Person Profile (Expanded)
  • Research & Related Budget
  • R&R Subaward Budget Attachment Form 5 YR 30 ATT
  • Research & Related Budget 10YR
  • R&R Subaward Budget Attachment 10 YR 30 ATT
  • R&R Multi-project 10 Year Budget
  • R&R Multi-Project Subaward Budget Attachment Form 10 YR 30 ATT
  • SBIR/STTR Information

Or an expiration date of 03/31/2020:

  • PHS 398 Cover Page Supplement
  • PHS 398 Modular Budget
  • PHS 398 Training Budget
  • PHS 398 Training Subaward Budget Attachment(s)
  • PHS Additional Indirect Costs Form
  • PHS 398 Research Plan
  • PHS 398 Career Development Award Supplemental Form
  • PHS Fellowship Supplemental Form
  • PHS 398 Research Training Program Plan
  • PHS Human Subjects and Clinical Trials Information
  • PHS Assignment Request Form
  • Biographical Sketch Format Pages (General and Fellowship)
  • Data Tables (used with Institutional Research Training Grant Applications)
  • PHS 416-1 Ruth L. Kirschstein National Research Service Award Individual Fellowship Application
  • PHS 416-5 Individual Fellowship Activation Notice
  • PHS 6031 Ruth L. Kirschstein National Research Service Award Payback Agreement
  • SBIR Funding Agreement Certification
  • STTR Funding Agreement Certfication
  • VCOC Certfication (used with small business applications)
