PTCAS Supplemental Application

Thank you for applying to the University of Texas Medical Branch (UTMB Health), School of Health Professions, Doctor of Physical Therapy Program through the Physical Therapy Centralized Application Service (PTCAS). Your Application ID is required to access the Supplemental Application. It can be found in the email correspondence you received containing the application link. If you need assistance obtaining your Application ID or assistance with this form, please contact The Office of Enrollment Services at 409-772-1215.

To access your Supplemental Application, enter your Application ID below and select the Submit button. Once submitted, your Application ID will be verified.

If you have any problems with the Online Supplemental Application please email us at EnrollmentServices@UTMB.EDU. If you have completed your Supplemental Application and have not yet paid your fee you may make an electronic payment here.
