Our CDC-required COVID-19 Vaccination Program Provider Site Visit concluded recently with no deficiencies found.
The representative surveyed the CLC Pharmacy and an off-site clinic location (UTMB Adult Primary Care, Highway 3, Suite 240), as it is a CDC and DSHS requirement that the reviewer observes vaccine preparation and administration.

I would like to recognize Jennifer Bledsoe for thoroughly preparing us for the site visit; Jonathan Aldaco, our pharmacy buyer/back-up vaccine coordinator for being able to detail out all answers asked of the representative for the pharmacy department; and RN Mandi Kennedy and MA Maytte Pacheco at the Adult Primary Clinic for being able to speak with the representative as well as provide observation regarding the storage, preparation and administration that takes place at the clinic.
