Medication-related osteonecrosis of the jaw (MRONJ) is a serious complication of antiresorptive and antiangiogenic therapies. In advanced cases, surgical resection with free tissue transfer is the standard of care, but determining adequate resection margins remains challenging. Although CT-detected osteosclerosis surrounding necrotic bone is commonly used to estimate disease extent, its correlation with histologic necrosis at surgical margins remains unclear.
