Texas Medical Board Faculty Temporary License

Important: Allow at least 60 days prior to your anticipated start date for UTMB processing and Texas Medical Board (TMB) review. TMB requires up to 45 days to process completed applications. GME requires the following items to be delivered to the Office of Graduate Medical Education at Suite 5.138 Rebecca Sealy, Attn: Veronica Lozano or Amelia Serrano. Please be sure that all NEW applications include the following:
☐ Completed UTMB Faculty Temporary License Application
☐ UTMB IHOP Policy and Acknowledgement Form
☐ UTMB Authorization to Review and Assist
☐ Current Curriculum Vitae
☐ Copy of UTMB Faculty Offer Letter (Assistant Professor level or higher)
☐ Copy of Medical School Diploma
☐ Copy of Residency Certificate(s)
☐ Copy of Fellowship Certificate(s) (if applicable)
☐ Copy of Visa Documentation (if applicable)
☐ Certified Translations if any documents are not in English

☐ $619.50 check/cashier’s check payable to Texas Medical Board

Additional Requirements for the Texas Medical Board
- Fingerprinting: Required for initial FTL applications. Instructions sent by TMB after submission. One-time requirement if renewed without lapse.
- Jurisprudence Exam: Must be taken through MyTMB account once application and fee are received by the TMB. Starting 9/1/2025, JP Exam retake is required every 5 years.
