Early Academic Referral Systems (EARS)
As we strive to help all our students excel in their academic goals, SSC has made an Early Academic Referral Systems (EARS) form. Faculty and staff can use the form to refer at-risk students directly to us so that students can get the resources they need
to succeed in their academic journey before they find themselves on probation.
A Student Success Center staff will follow up with you and give the necessary support that you need.
Complete EARS Form
Instructions on Completing the EARS form
- Click on the Complete EARS Form
- Sign in by providing your username and password
- Fill out the EARS form with all the necessary information.
- Click the Continue button to send your EARS form to the SSC staff.
- Check your email for a copy of your request.
** Please note that the copy of your request may not be immediately available, as it needs to be processed and assigned to an SSC staff member.
Presentation Request Form
The Student Success Center offers presentation sessions for UTMB faculty and staff to request for their students. These sessions are led by SSC staff and designed to support students' academic and professional success.
Instructions for Requesting a Presentation:
- Complete the Presentation Request Form below.
- Please submit requests at least two (2) weeks in advance of your desired presentation date.
- A representative from the SSC will contact you within three (3) business days of submitting the form. If you do not receive confirmation, please follow up via email: SSC@utmb.edu
