Expanding access to weight management: What the new Medicare GLP-1 Bridge program means for you

Navigating weight management options can  feel overwhelming, especially when balancing prescription coverage and out-of-pocket costs. For many Medicare beneficiaries, access to effective anti-obesity medications (AOMs) has historically been limited or financially out of reach.

To bridge this gap, Medicare has launched the Medicare GLP-1 Bridge program. Designed to expand access to  weight management treatments, the program introduces a $50 monthly copay for eligible patients.

What is the Medicare GLP-1 Bridge program?

The primary goal of the Medicare GLP-1 Bridge program is to improve access to GLP-1 receptor agonist medications for eligible Medicare beneficiaries with obesity or overweight who meet certain medical criteria.

Who qualifies?

To be eligible, beneficiaries must have Medicare Part D prescription drug coverage. Clinical eligibility is primarily determined by body mass index (BMI) before starting treatment:

  • BMI of 35 or higher: Qualifies for the Bridge program without having additional medical conditions.
  • BMI between 27 and 34.9: Requires one or more Medicare-defined weight-related health conditions (comorbidities). A physician evaluation, which may include laboratory or other tests, helps determine eligibility.

Important coverage exception: If you already have Medicare Part D coverage for a GLP-1 medication under a different FDA-approved indication, such as Type 2 Diabetes (Ozempic, Mounjaro), cardiovascular disease (Wegovy), MASH (Wegovy), or moderate-to-severe obstructive sleep apnea (Zepbound), you are not eligible for the Bridge program. Your coverage will continue through your existing Part D plan.

Exploring treatment options: Finding the right fit

The Bridge program covers four primary GLP-1 formulations:

  1. Injectable Wegovy
  2. Oral Wegovy
  3. Injectable Zepbound
  4. Oral Foundayo

Choosing between a daily oral medication and a weekly injection depends on your individual health goals, lifestyle, and personal preferences.

“When evaluating GLP-1 therapies, there is no one-size-fits-all approach,” Stark explained. “While injectable formulations have demonstrated higher weight-loss efficacy in clinical trials for individuals starting at a higher BMI, oral formulations offer a daily noninjectable alternative that fits into many patients’ daily routines, especially for those with lower BMIs, more modest weight-loss goals, or those transitioning to long-term maintenance therapy.”

  • Efficacy and weight goals: Injectable options generally offer higher weight-loss efficacy in clinical trials than oral formulations. Oral options are often well-suited for patients closer to a BMI of 30, whereas injectables may be recommended for those closer to a BMI of 40.
  • Administration and convenience: Oral Wegovy requires strict daily routines regarding timing and food intake to ensure proper absorption.Weekly injectables offer different delivery styles, ranging from Wegovy’s single-use auto-injector pen with a fully concealed needle to Zepbound’s reusable Kwikpen, which uses a small, attachable pen-needle for each weekly dose.

Beyond the prescription: A holistic approach to care

Starting a GLP-1 medication is just one part of a successful health journey. UTMB care teams emphasize combining prescription treatment with lifestyle support to help ensure safe, sustainable results.

“For older adults in particular, rapid weight reduction has to be carefully managed to safeguard muscle mass and bone density,” Stark said. “Pairing GLP-1 therapy with targeted resistance training and adequate dietary protein helps ensure that our patients are losing fat while preserving vital lean tissue.”

  • Muscle and bone protection: Providers recommend 30 minutes of strength training two to three times per week, along with meeting daily protein targets (typically about 90 grams for older adults) to help protect lean mass.
  • Accessible fitness programs: UTMB connects patients with local resources like the SAIL (Stay Active and Independent for Life) program, which offers free community strength and balance classes in League City and Galveston.
  • Ongoing clinical follow-up: Patients meet with their care team about every eight weeks to adjust medication doses as needed, monitor blood pressure, assess side effects, evaluate body composition to monitor muscle mass (at select UTMB locations), and ensure treatment remains aligned with personal goals.
