How to find a WebForm through the main page
To find a WebForm through the main page, please login at the following link with your UTMB credentials: https://webforms.utmb.edu/frevvo/web/login
Once you have logged in, click on My Tasks. This page may take a moment to load. Once it does, find the WebForm you are looking for in the left-hand column. Click on the red triangular play button to open and complete
the form.
