Adults age 55 and older living in the Galveston area who have been diagnosed with hypertension (high blood pressure) are invited to participate in the Health Coaches for Hypertension Control (HCHC) program offered by UTMB Patient Education Programs.

This free, evidence-based program, developed by the Clemson Institute for Engaged Aging, helps participants better manage high blood pressure through practical lifestyle strategies and personalized support. Trained health coaches provide guidance on nutrition, physical activity, stress management, medication adherence, and other healthy living topics.

Participants will meet with a Certified HCHC Coach Leader for 1.5 hours each week over eight weeks and will receive a free blood pressure monitor along with educational materials to support their health goals.
