External Phone Services
UTMB Customer Service will order local phone lines for campus or offsite locations from the appropriate telephone
carrier. A quote for installation and monthly costs will be provided to the department at the time of the request.
To request this service, complete the Telephone, Data Video Request Form
with the following information:
- Department name
- Physical address with zip code
- Contact and phone number
- Type of service requested (i.e., analog line, circuit)
- Billing PeopleSoft Chartfield
