Home-Based Primary Care Program has been nominated for The Galveston Daily News Readers’ Choice Awards!

The UTMB Geriatric Home-Based Primary Care Program team has been nominated for the Galveston County Daily News 2026 Readers' Choice Awards in the Best in Home Health category, an honor that recognizes the program's commitment to providing exceptional, patient-centered care to older adults throughout our community.

The Geriatric Home-Based Primary Care Program brings comprehensive primary care directly into patients' homes, ensuring that individuals with complex medical needs receive high-quality care in the environment where they are most comfortable. Through collaboration, compassion, and innovation, the team helps patients maintain their independence while improving access to care and overall quality of life.

Advanced practice providers (APPs) are an integral part of the program's success. Their clinical expertise, dedication to patient advocacy, and commitment to building meaningful relationships with patients and families exemplify the highest standards of the profession. By delivering care directly to patients' homes, the team provides a level of personalized attention that extends far beyond the traditional clinical setting.

Voting is open through August 27, and winners will be announced on September 26. Colleagues, patients, families, and community members are encouraged to show their support by visiting the Readers' Choice voting website and casting their vote for the Geriatric House Calls team.
