Event Function Approval Form

The Event Function Approval Form must be completed and sent to the President's Office for approval on any UTMB Health event with a budget over $2,000.

Alcohol Service Request Form

All UTMB Health events held on or off campus that wish to serve alcohol must submit this form to the President's Office for approval prior to event date.
