Event Function Approval Form

The Event Function Approval Form must be completed and sent to the President's Office for approval on any UTMB Health event with a budget over $2,000.

Alcohol Service Request Form

All UTMB Health events held on campus that wish to serve alcohol must submit this form to Tilly Clark - Office of the President at taclark@utmb.edu for approval prior to event date.
