iDSMES Registration

Program Eligibility

To be eligible for referral to the iDSMES program, participants must:

  • Be at least 18 years old, and
  • Be overweight (Body Mass Index ≥25; ≥23 if Asian), and
  • Not pregnant, and
  • Have a previous clinical diagnosis of type 2 diabetes, or
  • Have a blood test result within the past year:

Before you join a diabetes class, you MUST fill out this APPLICATION FORM.

All dates are tentative provided at least 10-15 participants register for each class.
