Developed at Stanford University Patient Education Research Center as a collaborative research project with the Northern California Kaiser Permanente Medical Care Program, this program is proven to help people with diabetes learn the necessary skills to manage their diabetes through healthy eating, stress management, and more. This program is made available for people 65 years of age and older with diabetes. Meet with your Certified DSMP Leader for 2.5 hours once weekly for 6 weeks.
