iDSMES Program for Diabetes

What is Diabetes? ¿Qué es la diabetes? 

Diabetes is a chronic (long-lasting) health condition that affects how your body turns food into energy. Your body breaks down most of the food you eat into sugar (glucose) and releases it into your bloodstream. When your blood sugar goes up, it signals your pancreas to release insulin. Insulin acts like a key to let the blood sugar into your body’s cells for use as energy.

Intensive Diabetes Self-Management, Education and Support

This program aims to augment the diabetes self-management among people with diabetes through improving related knowledge, skills and self-efficacy in group sessions of 10-15 patients per class. Continue your diabetes education beyond the Standard DSMES. For you, we compiled a one-year educational program (iDSMES) that will be implemented by our certified Diabetes Care and Education Specialists, Lifestyle Coaches and Journey for Control’s U.S. Diabetes Conversation Map® trainers. The program includes three components:

  • Phase-1 (months 1-2) Standard DSMES: Refresh your knowledge about diabetes, self-care and nutritional management and work with our certified diabetes care and education specialists and dietitian to achieve your goals.
  • Phase-2 (months 3-8) Lifestyle Coaching: Learn fundamental skills to make lifestyle changes with our lifestyle coaches. Improve your skills of dealing with environmental elements and psychological and emotional issues that may hinder your achievement of lifestyle goals and maintenance of a long-term change. Meet for 1h-interactive once monthly sessions.
  • Phase-3 (months 9-12) Journey for Control’s U.S. Diabetes Conversation Map®: Play, discuss and have a free-flowing dialogue with peers and trainers learning about diabetes and self-care management. Meet for 1h-interactive once monthly sessions.
