PreventT2® Program for Medicare Beneficiaries with Prediabetes

Program Eligibility

Our Medicare Diabetes Prevention Program (MDPP) can cut your diabetes risk by 71%. You are eligible for MDPP if:

  • Be enrolled in Medicare Part B through Traditional Medicare or Medicare Advantage Plan, and
  • Be overweight (Body Mass Index ≥25; ≥23 if Asian), and
  • Have no previous diagnosis of type 1 or type 2 diabetes, and
  • Have no diagnosis of end-stage renal disease, and
  • Have not received any MDPP services before, and
  • Have a blood test result in the prediabetes range within the past year:

Before you join a prediabetes class, you MUST take our DIABETES RISK TEST.

All dates are tentative provided at least 10-15 participants register for each class.
