Dr. Vinod Kaushik, board certified geriatrician, and his colleagues provide a community-based service for Galveston County, devoted to the care and treatment of the elderly homebound patient. UTMB Geriatrics Home Based Primary Care provides you an office experience in the comfort and privacy of your home by geriatric specialized healthcare providers.
Our services include the following:
- Monitoring long term treatment of chronic conditions
- Medication management
- Temporary visits after hospitalization before patient is able to return to clinic
- Coordination of care with home health agencies
- Basic blood work, procedures, and imaging in the home
- Coordination of social needs
- Coordination with UTMB health specialists and hospital
- Palliative Care
Eligibility for the Home Based Primary Care Program include the following:
- Patients over 65 years of age
- Difficulty transporting to clinic appointments due to cognitive or physical decline
- History may include stroke, severe decline in mobility, Parkinson’s, dementia, COPD, congestive heart failure, oxygen dependency
- Resident of Assisted Living or Memory Care Unit
