Honeybee Long-term and Transitional Care Clinic (HBC)

An interdisciplinary team of one pediatrician, OT, PT, Dietician, and Social Work (either on-site or by referral).Our services include, but are not limited to:

  • Primary care medical home model to offer services for children with special needs, including and beyond usual pediatric age range.
  1. Patients who are > 18 years old with chronic illness, neurodevelopmental disabilities, feeding difficulties, failure to thrive/poor weight gain, and/or technology dependence.
  2. Patients who are on hospice or appropriate for hospice
  3. Patients who have been discharged from the CLC hospital and do not already have a relationship with Pediatric Complex Care
  4. Patients whose parents directly request the Honey Bee Clinic or Dr. Gerik’s team
  5. Patients referred via Christina Sullivan Foundation
  6. Special Care Unit discharges from CLC 
  • Assessment of hospital-to-home transition for children discharged from Clear Lake Campus not established with Complex Care.
  • Pediatric hospice evaluation and services         
  • Assistance with the transition of care to other adult physicians/specialists after age 18, including Christina Sullivan Foundation patients/families.
  • Special Care Unit discharges (including SGA) from CLC.
  • Therapy needs assessment for OT and PT developmental services in the clinic for the population as outlined above. Referral to an outside speech therapist for evaluations as needed.
  • Care Coordination for private duty nursing, supplies, and durable medical equipment after discharge by HBC pediatrician or after enrollment in Dr. Gerik’s Clinic.
  • Feeding/nutrition evaluations for patients as outlined above.

    This clinic provides both Primary Care and Specialty Care services to children and youth from birth through 21 years of age. All services are provided in a family-centered setting with specialized education and support to optimize care and outcomes.
    This clinic is a state-of-the-art facility. Faculty and resident pediatricians are present in this location, providing care for infants and children from the newborn period through adolescence. As in our other locations (Island Pediatrics East and West), there are opportunities for families to participate in clinical studies, including those for vaccines.
