Children's Health Service Complex Care

 

Services Offered

Our goal is to improve the lives of children living with, or at risk for, chronic health conditions through the following ways:

  • Provide detailed developmental, feeding and nutritional, educational, equipment, and care coordination evaluations with a multidisciplinary team.
  • Recommend a diagnostic workup according to the American Academy of Pediatrics standard of care.
  • Care for the effects of chronic health conditions using medication, therapy, nutrition, and/or equipment services.
  • Refer to appropriate medical and community resources, when needed.
  • Provide anticipatory guidance and complex disease home management education to patients and caregivers.
  • Provide care coordination of outpatient and community resources.


Service Settings

  • We provide outpatient services in two locations (Galveston and Bay Colony) either through a one-time consultation or an ongoing care model.
  • We can assist with medical needs while hospitalized through inpatient consulting services.


Discharge from the Pediatric Complex Care Program

  • Once a patient’s medical needs have stabilized, the family and Complex Care Team can discuss the full transition of medical services back to the child’s PCP.
  • Patients will be transitioned from our general clinic services by age 21. Adults with mobility equipment needs may still access clinic for those services only with the expectation all other medical management will be provided by their PCP.
  • Children should keep regular well-child checks and as-needed sick visits with their PCP while enrolled with the Pediatric Complex Care Program.
